An investigation has been launched by All Hallows Catholic High School after the details of pupils were found in a car park.
Reply slips from a school trip organised by the school in Penwortham were found scattered around in Liverpool ONE.
The slips contained pupilâs names, pupil form numbers and parent names and signatures.
One document included further details, with the full address of a parent and pupil.
The school had been on a day trip to Liverpool Echo Arena but it is unclear how the forms ended up at the Liverpool shopping centre’s car park.
Lancashire County Council confirmed the paperwork may have been stolen from the handbag of a teacher during the trip.
The county council said the slips may have been taken after thieves thought money was attached to the forms.
All other documents were filed and signed for on the return of the trip.
Chris Riding, headteacher at All Hallows Catholic High School, said: âWe regret this unfortunate incident and are still looking at how it could have occurred.
âFortunately the reply slips only contained the name and class of each pupil. There is no indication that this information could be used inappropriately however we take the security of the information we hold seriously and we we will remind staff of their responsibilities.â
All documents have been returned to the school.
